Models for transitioning adolescents with HIV to adult care?

Anyone know of organizations or people working on transitioning adolescents with HIV from pediatric to adult care? I am also looking for models to assess readiness to transition, as well as guides for implimenting a successful transition program, either in the US or other nations.

See below the link for the document "Guidance on transition and long term follow up services for adolescents with HIV infection acquired in infancy " prepared by CHIVA ( children's HIV association)

it's a comprehensive review.Also it highlights a variety of models of transition and provides practical steps that are useful in the transition process.

Also you may want to check the website of AETC (Aids education and training center) National resources center
http://www.aidsetc.org/
they have some useful tools like "the adolescent transition workbook",the "health literacy checklist " and others that clinicians can use to monitor and assist their patients during the transition.
