Elizabeth Mitchell on the Role of Providers, Payers, and Purchasers in Incentive Design

Today we are delighted to welcome Elizabeth Mitchell as a Keynote Speaker for this Breakthrough Opportunities event on Designing Provider Incentives with the Commonwealth Fund.

Ms. Mitchell is President & CEO of the Network for Regional Healthcare Improvement, a Guiding Committee Member of the Health Care Payment Learning and Action Network (LAN), a member of the Institute of Medicine’s Consensus Committee on Core Metrics for Better Care and Lower Costs, and chairs this committee’s Implementation Task Force.

In this video, Ms. Mitchell discusses the key roles of providers, payers, and purchasers in designing effective incentive programs.

How can providers be responsible for social determinants (nutrition, education, housing etc.) of health that inform issues of adherence and optimized outcomes?

Dear Mitchell
I would like to any bring a totally different approach to the challenges you are facing and what you mentioned in this video in the following way.
1. First and foremost i think the moment you mention “Incentives “ to a Physician , health Insurance agencies or any stakeholders in this effort it immediately focuses the discussions towards money distribution or expenses. This will divert the whole attention away from what really we want to achieve.
2. Before starting on this effort the first thing we need is to get information on the intrinsic motivations of the physicians. The whole physicians’ network should be subjected to data analytics to understand what motivates them to behave in a certain way. Then that will give you valuable insights on the Physicians motivational aspects that can drive your team’s conversation accordingly. I certainly feel that everyone loves to be recognized as a brand or for what they achieved. (Just think about yours and my motivation for this effort). That is one benefit for the physicians to do a good job.
3. So now you provided a incentive to physicians by giving them the above based on their Intrinsic desires.
4. Since money is still needed for this effort (for all the payments) I propose the following idea. This is also useful for taking care of health needs of the poor across US I guess.
a) A Non profit government agency should be created.
b) A donation box should be placed across major shopping malls, retail stores, US Posts offices, government offices , private and public enterprises , hospitals, physicians offices and where ever it is possible for anyone to drop a dollar and that should be regularly collected by the above agency –traditional way
c) Create an App that can be automatically installed on every mobile on the planet with a payment gateway to donate a dollar/local currency to a account created by government. The money should be exempted from taxes. The tag line that needs to be promoted should be “wellbeing of the planet and its people Please make it happen” –modern way.
d) This money should be used for incentivizing the services of all the stakeholders.
e) Then coming to providing quality health care for all the poor and needy across US without any health insurance I propose the following.
1) Government should match the above donations on a quarterly basis so that this pool of money can become surplus.
2) All the uninsured medical claims should be going to this agency for reimbursements. The agency should send the summary of the expenses incurred by each physician in delivering service , summarized annually to IRS. This becomes the proof for IRS to know which services are given to the uninsured patients and which physician’s claims can be matched from tax exemptions.
3) The Physicians on their part should be able to be exempted from income tax for the same amount when they file for taxes.

After thoughts here.
Sustainability of any global challenge (whether it is meeting the rising costs of health care or solving global warming) is only possible in a very simple, authentic and sincere collaborative efforts by all the humans of the planet.
But to trigger the above emotions in human beings I felt we need this to be promoted carefully with the right words and my contribution for that is “Well being of each one of us and the planet , let’s just make it happen”

A wonderful reminder of four main facts:
1. evidence-base accumulation is a collaborative effort, we should collectively gather more rigorous data.
2. collaborative efforts also mean more collaborative projects involving providers, patients, and all stakeholders in general.
3. genuine support, engagement, and flexibility are key features when designing incentives to healthcare professionals.
4. incentives should be designed to capture overall effects, having in mind the whole healthcare system.
